Eligibility Criteria Document
Delivery must be made free of cost to WAPDA Hospital Rawalpindi within the specified time. Delay will result in liquidated damages @ 0.067% per day (or part thereof). Rates must be inclusive of all taxes (GST) and valid for 30 days. All items must comply with relevant international standards (ISO/CE/FDA, where applicable). Substandard, expired, or damaged items will be rejected at supplier’s risk and cost.
Payment (100%) will be made after complete supply, subject to submission of required documents as per WAPDA/PPRA rules. Applicable withholding taxes will be deducted as per prevailing government rules. Sales Tax Invoice (triplicate). Warranty Certificate (as per Drug Act 1976, if applicable). Shelf-life Certificate (if required).
Active ATL status proof (Income Tax & Sales Tax). Professional Tax Certificate (Punjab). Drug Manufacturing/Sales License (if applicable). Undertaking that payment has not been previously claimed. Bill of Entry & import documents (if applicable). Delivery Challan. Inspection Report. Bank Account Details. Any other document required under WAPDA/PPRA rules.
